To apply for other programs at UCLL, an Application Form needs to be completed:

• If you are interested in signing your child up for an Evaluation, Individual Therapy, Preschool and Communication Therapy (PACT), or Reading to Learn, please download this application*

• If you are interested in signing yourself up for an Evaluation or Individual Therapy, please download this application.

*Please note: If your child has had any other therapy or evaluations completed at school or by another specialist, please include a copy of the report(s) with the application.

We will review all submitted information and appropriate candidates will be invited to participate in the programs.

Available spaces are filled on a first-come, first-served basis in response to the invitation.
